The stairs test: 60 seconds that predicts a lot

A stopwatch and four flights of stairs give a rough read on aerobic capacity. Under 60 seconds is a good sign; over 90 is a prompt to investigate. What the test measures, what it misses, and how to retest it properly.

Four flights, one stopwatch

Find a staircase with four consecutive flights — roughly 60 steps. Warm up with two or three minutes of easy walking. Then climb at a fast, steady pace: no running, no taking two at a time, hands off the rail except for balance. Start the stopwatch on the first step and stop it on the top landing. That is the whole test. No lab, no mask, no watch.

It comes from cardiology, not fitness marketing. Researchers in Spain timed 165 patients with known or suspected heart disease up 60 stairs, then compared the result against formal treadmill exercise testing. Climb time tracked measured capacity closely enough to work as a screen: under about 45 seconds generally meant good capacity; over 90 seconds meant poor capacity.

What the number means

Capacity here is measured in METs — multiples of your resting energy burn. Sitting is 1 MET. Ten METs is roughly the effort of a fast uphill hike or a steady run. In the stair data, climbs under about 45 seconds pointed to 9–10 METs or more; climbs over 90 seconds pointed to under 8. The popular 60-second line sits between those two markers — comfortably clear of the slow zone.

Why care about a MET? Because exercise capacity is one of the strongest long-term health predictors clinicians have. In a study of 6,213 men followed for six years, each extra MET of capacity was associated with roughly 12% better survival (Myers, NEJM 2002). The stairs test is a crude ruler, but it is measuring an important quantity.

What it cannot tell you

A slow time is not a diagnosis. Knees, unfamiliar stairs, a heavy bag, a poor night, or simply being new to training will all add seconds. A fast time is not a clean bill of health either — it estimates capacity, not what your arteries look like. Symptoms outrank the stopwatch: chest tightness, dizziness, or breathlessness out of proportion to the effort are reasons to see a doctor whatever the time says.

One climb is weather. The trend is climate. A single attempt bakes in that day's sleep, stress, caffeine, and stairs, so retests on the same staircase, weeks apart, are what carry signal — direction matters more than any one reading. Skip test days when you are ill, and if you have a known heart condition or get chest pain on exertion, talk to a doctor before testing yourself at all.

How to move the marker

Aerobic capacity responds to consistent volume more than to heroics. Endurance research consistently finds that keeping most sessions easy — conversational pace — with a small share of genuinely hard work beats going moderately hard every time. Weeks of unremarkable, mostly easy cardio move the retest more reliably than the occasional all-out effort does.

Retest every four to six weeks. Same staircase, same warm-up, same rules, ideally at the same time of day. Write the time down. Expect the curve to flatten as you improve: getting from 90 seconds to 70 tends to come quickly; getting from 50 to 45 does not. Both are progress.

What if my time is over 90 seconds?

Treat it as a prompt, not a verdict. Rule out the mundane first: unfamiliar stairs, illness, joint pain, no warm-up. If a fair attempt is still slow — or the climb causes chest symptoms or disproportionate breathlessness — book a check-up. Otherwise, build easy aerobic volume for six weeks and retest on the same staircase.

Do I need exactly four flights?

The research protocol used roughly 60 steps. If your building offers three or five flights, the published thresholds stop applying, but the test still works as a personal benchmark. Pick one staircase, keep the protocol identical every time, and compare yourself only against your own previous attempts.

Is this as accurate as a lab VO2 max test?

No. A lab test measures oxygen uptake directly; the stairs test infers capacity from one timed effort, with noise from technique, the stairs, and how you slept. Its advantages are cost and repeatability: you can run it monthly for free. Use it for trend, not for precise numbers.
